Custom Facebook Questionnaire

To get started on your Custom Facebook campaign, we will need you to complete and submit our initial questionnaire. After we receive your completed questionnaire, you will be assigned to an account manager who will contact you to introduce themselves and review your answers.

At this time, we will also begin designing and optimizing your social media channels. Once complete, links will be sent by email for your review.

Thank you for your recent purchase! We are excited to begin designing your practice Facebook page but will need to know some information first. Below are a few questions that will help us design your Facebook exactly how you would like! Please take a moment to fill out this Questionnaire. Do have an existing Facebook Account?
- YES - You Will need to Add PBHS as a Manager on your Page - See below
- NO - You will need to create a personal account in order to access your business page. - Once you have, please provide us with the email address of the account so we can make you a manager.
To add PBHS into your existing Facebook Page:
1. Sign into your Facebook account and downward pointing arrow at the top right.
2. Select Use Facebook As (your Business Page)
3. Select "Settings"
4. Select "Page Roles"
5. Add PBHS as an admin by typing “[email protected]” and choosing “Manager”.
