truForm Administration

In order to view and print truForm submissions, you must first login to the truForm administration page. Login by clicking here.

On the login page, enter in your truForm user name and password. (The user name and password should have been sent to you via email. If not, please email us.) Click the Login button.

This will transfer you to an admin screen which lists the client submitted truForms sorted by the categories: Patient Registration, Referral and Miscellaneous. Under each of these categories, the submitted truForms will be listed giving the first and last names of the client and the date the form was submitted. The listing is sorted by date received and new entries are bold. You can sort the entries by last name by clicking on the Last Name link in the darker blue strip at the top of the section. To re-sort by submission date, click on the Submitted link in the darker blue strip at the top of the section.

On the right side of each entry, under the Actions heading, there are three buttons -- open, delete and save. Click the open button to view and then print your truForm PDF file. When you open your truForm, Adobe Acrobat opens the form with the data completed by your patient. You can now print your chart by using the print icon on your toolbar or ...File ...Print. Close the pop-up window to return to the admin screen.

To save the client form to your desktop, click on the save button.

When you no longer need the PDF file, on the admin page, click the delete button to delete the truForm PDF file (the form is placed in an archive folder). A pop-up window will prompt you to verify the deletion. Click OK.

To retrieve a patient file that was previously saved, click on Recover Registration Forms or Recover Referral Forms.

Our HTML based referral forms now allow for the secure uploading and retrieval of radiographs. When a referring doctor submits a radiograph, it is placed in a folder on our secure server. When you view the patient file, the radiograph is attached as a final page to your PDF. You can retrieve these stored radiographs by using the View Patient Xrays link. The time and date will match that of the referral form.

When you are finished viewing your truForm PDF files, close the window in which your truForm resides. The truForm system will automatically log out.

Printing X-rays

To print over-sized files, such as panoramic x-rays, through the browser you need to set the page orientation to "Landscape" and reduce the image size by an appropriate setting. The file name, which includes the patient name, is printed on the x-ray in Firefox 2 and Internet Explorer 7(IE). These instructions are for an HP4100 printer. The actual dialog names and setting locations will vary depending on your printer make and model.

The first step after opening the x-ray in your browser is to open the print dialog box. In IE this is the printer icon in the upper right hand portion of the browser. The dialog is located in the File menu in Firefox. Select the Preferences button for IE or the Properties button for Firefox. In the dialog box that opens, select the Effects tab and then set the % of Normal option (IE) or the Scale To Fit option (Firefox) in the Resizing Options section. For printing a large panoramic x-ray in IE the % of Normal setting should be 30%.

Select the Finishing tab and set the Orientation option to Landscape. Click OK then Print.

If your printer settings do not allow the file name to be printed on the page please have your referring dentist add the patient name to the x-ray before it is uploaded.
